1The problem
How clean does this need to be?
You can't sterilize everything after every patient — a tubehead won't fit in an autoclave. So the Spaulding classification sorts every item by risk: how far into the body does it go? Penetrates tissue, touches mucous membranes, or only touches intact skin — each tier gets its own level of cleaning.
Under it all is standard precautions: treat every patient as potentially infectious, so the routine never changes. Tap through the tiers below — and watch for the radiography trick.
2The instrument

That's Spaulding. The radiography trick: nothing in dental radiography penetrates tissue, so there are no critical items. Sensors and holders are semi-critical; the PID and tubehead are non-critical. And sensors can't take heat — so they get a barrier + disinfectant, not the autoclave.

Standard precautions
Treat every patient and all body fluids as potentially infectious. Same protocol for everyone — no exceptions, no guessing.
Critical
Penetrates soft tissue or bone → must be sterilized. In dental radiography, nothing qualifies — the classic trick answer.
Semi-critical
Touches mucous membranes without penetrating → sterilize or high-level disinfect. The digital sensor and XCP holder.
Non-critical
Contacts only intact skin → intermediate/low-level disinfect or barrier. The PID, tubehead, exposure button, chair.
Digital sensors
Heat-sensitive — never autoclave. Use an FDA-cleared barrier plus an intermediate-level disinfectant between patients.
Spaulding sorts items. But the room itself needs the same before/after discipline. Before a patient sits down, cover every clinical contact surface — anything your gloved hands or contaminated instruments will touch mid-procedure: control panel, exposure button, tubehead/PID, chair back and headrest, mouse/keyboard if used chairside.
Before the patient
Place barriers (plastic wrap, tube sleeves, headrest covers) on every clinical contact surface you'll touch during the appointment.
After the patient
Remove barriers with gloved hands without touching what's underneath. Disinfect anything touched without a barrier, or any spill, with an EPA-registered intermediate-level disinfectant — then place fresh barriers for the next patient.
Clinical contact vs. housekeeping
Clinical contact surfaces (touched during care) get a barrier or disinfection between every single patient. Housekeeping surfaces — floors, walls, sinks — are cleaned on a routine schedule, not per patient, since care doesn't usually touch them.
6Beat the exam
How DANB will actually ask this.
It hands you an item and wants its tier, or names an agency and wants its job. Match the words:
"penetrates soft tissue or bone"→critical — but none in radiography!
"sensor / XCP — touches mucosa"→semi-critical
"PID / tubehead — intact skin only"→non-critical
"digital sensor between patients"→barrier + disinfect (never autoclave)
"control panel, chair — covered before the patient sits down"→surface barriers on clinical contact surfaces
A digital intraoral sensor is placed in the patient's mouth. Under the Spaulding classification, it is considered:
Semi-critical. It contacts mucous membranes but doesn't penetrate tissue. Ideally sterilized or high-level disinfected — but since most sensors can't be heat-sterilized, the practical standard is a barrier plus an intermediate-level disinfectant.
